Heart to Home

Heart to Home is Traditions Health’s comprehensive, patient-centered cardiac management plan for individuals with cardiac disease, allowing patients’ symptoms to be managed at home with fewer crisis situations, decreased hospitalizations, and improved quality of life.

INSPIRE

INSPIRE is our comprehensive, patient-centered pulmonary management program for individuals living with pulmonary disease. This program allows a patient’s symptoms to be managed at home with fewer hospitalizations and an improved quality of life.

Next Step

Tradition’s Next Step Program takes in-home rehabilitation to a new level. When a patient reaches a certain level of autonomy with traditional home health therapies, our therapists utilize equipment in the home to take them to the Next Step of maximum independence!

Next Step

Readmit Rescue

The Traditions Health Readmit Rescue program focuses on activities that emphasize collaboration of care with individualized goals and interventions to meet each patient’s care needs. This involves a multi-disciplinary approach to care between acute and post-acute care facilities, physicians and non-physician providers, and the home health care team members.

Wound Care

Our team collaborates with care providers to determine the best course of treatment to assist with wound healing. We have certified wound care nurses that have received enhanced education on wound management and passed a certification exam. These nurses may be available for in person visits or by consultation with a local nurse.
