President/Chief Executive Officer
David Klementz joined Traditions Health as President and CEO after spending more than 15 years with Encompass Health, formerly HealthSouth, where he most recently served as Chief Strategy Officer and was previously an SVP of Operations. Prior to joining Encompass, he served as SVP and CFO of Progress Rail, a $1.3B rail services business, where he was responsible for strategic and business planning, financial performance and leadership of the company’s financial, accounting and information technology functions. He has also held the position of manager of business development and planning for Progress Energy, vice president of finance and accounting for Analytical Sciences, Inc., and division controller for the Oracle Corp. A certified public accountant, Klementz received his bachelor’s degree in finance from James Madison University, his certification in procurement and contract administration from the University of Virginia, and completed Duke University’s Fuqua School of Business Leadership Development Program.
Chief Financial Officer
Alan is a CPA and has more than 20 years of healthcare finance experience. He joined Traditions Health in 2018 after serving as CFO for Camellia Home Health and Hospice for 11 years. Prior to his time with Camellia, Alan worked in public accounting, hospital finance, and held a regional position within another home health and hospice agency. He has previously served on the Home Care & Hospice Financial Managers Association’s (HHFMA) Advisory Board, which provides guidance to the National Association for Home Care & Hospice (NAHC) board of directors and the NAHC membership on financial matters in the home care industry. Alan has also served on Enterprise Fleet’s National Customer Advisory Board and on the Southern Miss Alumni Association’s Board of Directors. He is a graduate of the University of Southern Mississippi.
Alan and his wife, Angie, reside in College Station, Texas. They have two children.
Mariana A. Pope, JD, MPH
Chief Compliance Officer
Mariana is a performance-driven professional with over 10 years’ experience in health care compliance, including HIPAA privacy and security, billing compliance, and corporate compliance. Prior to joining Traditions Health, Mariana served as Director of Compliance and Privacy for Texas Children’s Hospital.
Mariana earned her Bachelor of Arts from the University of Houston, her Master’s in Public Health from the University of Texas Houston Health Science Center, and her Doctor of Jurisprudence from the University of Houston Law Center.
Mariana and her husband, Cameron, reside in Houston, Texas. They have four children.
Mark Sabella, Ph.D.
Chief Commercial Officer
Mark joined Traditions Health in August 2021. He is an accomplished healthcare executive with a proven track record for delivering results and building high-performing teams. Mark was previously with UCB pharmaceuticals working as the VP, Head of Asset & Commercial Strategy providing leadership for a global team responsible for bringing a product currently in development for rare neurodegenerative disorders to patients. Prior to this at UCB, Mark served as the Head of Operational Excellence for North America providing strategic leadership for a variety of teams including: Advanced Analytics, Forecasting, Performance Training, Incentive Compensation, Sales Force Effectiveness and Omni-channel Marketing.
Mark has also held progressive roles at Mallinckrodt, Sanofi-Aventis and Bayer pharmaceuticals over a span of 20 years and has served as a leader in a variety of commercial functions including: Sales, Sales Leadership, Marketing, Operations and Analytics.
Chief Development Officer
Tarik has more than 30 years of professional leadership experience, with more than 15 of those years in the home care sector. Prior to joining Traditions Health in 2019, Tarik was President/CEO of Centrix Health Resources - an organization offering hospice, home health, durable medical equipment, private duty, and physician services.
Tarik attended the University of Minnesota, where he majored in management and finance.
Tarik and his wife, Audrey, reside in Mesa, Arizona. They have five children.
Executive Vice President of Clinical Services
Pat joined Traditions Health as Senior Vice President of Clinical Services, Performance and Education in November 2020.
She has over 25 years of clinical healthcare experience, with 16 of those years dedicated to home health and hospice care. Prior to joining Traditions, Pat served as Chief Operations Officer and Chief Compliance Officer for Camellia Healthcare which was acquired by Encompass Health and where she served as National Regional President for the hospice division.
Crystal Mineo Diaz, PHR, SHRM-CP
Senior Vice President of Human Resources
Crystal joined Traditions Health Care in 2011. Her background includes responsibility for employee benefits, employee relations, payroll system integration, talent acquisition, mergers and acquisitions, and management. Crystal is a Certified Professional in Human Resources (PHR) and Society of Human Resource Management Certified Professional (SHRM-CP).
Crystal has a Bachelor of Science in psychology from Sam Houston State University and a master's degree in human resource management from Tarleton State University.
Crystal and her husband, Sonny, live in College Station, Texas. They have three children.
Tony Maxwell, PA
President, Central Region
Tony Maxwell has over 30 years’ experience in the health care setting. Prior to joining Traditions Heath, he served as the Regional Vice President of Operations for Encompass Health, Regional Director of Operations for Visiting Physicians, and as an Assistant Clinical Professor at the University of Texas Health Science Center in San Antonio. Tony began his career as a Physician Assistant 25 years ago, as an officer in the United States Air Force.
President, North Region
Freeman has more than 25 professional years of Health Care experience. Prior to joining Traditions Health, he served as COO of Harbor Light Hospice.
He attended DePaul University where he received his degree in psychology. In addition to his many accomplishments, Freeman is conversant in American Sign Language.
Freeman and his wife, Sean, reside in Naperville, Illinois. They have one child.
President, Southeast Region
Chad Morton joins Traditions Health with more than 20 years of healthcare management experience, 12 of those having been served in the home health sector. Chad was most recently the Senior Vice President of Operations for Encompass Home Health’s Mid-Atlantic Region. Prior to his time with Encompass, Chad served in various leadership roles including Regional Vice President of Operations and Sales for Camelia Healthcare and Managing Director Of Therapy Services for Amedysis Home Health.
Chad is a graduate of the Medical College of Virginia where he earned his degree in Occupational Therapy.
Chad and his wife Mari-Ruth reside in Tennessee. They have one child.
President, Southwest Region
Becky Tooker has more than 20 years of leadership experience in the hospice industry. She joined Traditions Health in 2019 as Vice President of Operations for their East Division after serving as Area Vice President of Operations, Area Director, Executive Director and Vice President of Sales/Business Development with national providers serving the post-acute care markets. Becky attended Spencerian College, where she earned her degree in nursing. She later received her Bachelor of Business Administration and Master of Healthcare Administration from Capella University.
Becky and her husband, Travis, reside in Palestine, Texas. They have 3 children and 6 grandchildren.
Jo Ann Mack
President, West Region
Jo Ann Mack joins Traditions Health with more than 30 years’ experience in the home health, private duty, and hospice industries. She worked with VITAS for 16 years as a Senior Vice President of Operations. She then joined VNA Health in Santa Barbara as Chief Operating Officer. Following her time with VNA Health, she was the Chief Operating Officer for Mission Health Care.
Jo Ann and her husband, David, reside in Los Angeles County. They have three children and three grandchildren.
