About Traditions Health

Since being founded in 2008, Traditions Health has maintained a patient-focused and compassion-driven care approach to Palliative Care and Hospice services. Palliative Care

Our palliative care team provides essential care for the patient with a serious or life-limiting illness that is still pursuing curative treatment. Our patient-centered care team provides the patient and family with symptom management as well as emotional and spiritual support as they navigate life with an advanced illness.

Hospice

When a patient is diagnosed with a life-limiting illness, hospice allows them to remain in the comfort of their own home surrounded by their loved ones. Our hospice service combines medical care and support for emotional and spiritual needs all in an effort to provide the best possible quality of life in their final days.
