Our Readmit Rescue program focuses on activities that emphasize collaboration of care with individualized goals and interventions to meet each patient’s care needs. This includes prevention of return visits to the Emergency Department (ED) or hospital. Traditions Health team members will work with your physicians, other care providers, and your caregivers to achieve this goal.

Keeping patients healthy after hospitalization leads to improved patient outcomes. Our solution:

Engagement with you, your caregivers, and facility care team prior to discharge
Individualized plan of care and assessment of risk for return to emergency department
Identification of resources needed for smooth transition
Timely admission assessment in the home with provider collaboration for plan of care
Daily check-ins with you or your caregiver during first 14 days of home health service
