INSPIRE is Traditions Health’s comprehensive, patient-centered pulmonary management plan for individuals living with pulmonary disease, allowing patients’ symptoms to be managed at home with fewer hospitalizations and an improved quality of life.

Plans of Care Tailored to You

INSPIRE focuses on early identification and management of symptoms and provides a personalized plan of care tailored to the individual needs of the patient that includes:

  • A comprehensive assessment
  • A personalized plan of care developed with the physician, including:

    -A Pulmonary Action Plan which outlines both pre-emergency and emergency care of symptoms

    -A practical, user-friendly Pulmonary Journal, which includes patient/caregiver monitoring of symptoms
  • Regularly scheduled home visits, telemedicine visits, and phone visits
  • Access to a Traditions’ registered nurse 24/7 for crisis management and support
  • Access to a Registered Respiratory Therapist for home visits, and/or phone consultations, if available
  • Individualized pharmacologic and non-pharmacologic interventions to manage symptoms and decrease emergent situations

    -Includes medications, nebulizer treatments, and other modalities to address comfort and quality of life. May also include oxygen therapy as ordered by the physician.
  • Specialized medical equipment, including personal oxygen saturation monitors and incentive spirometers, will be provided to patient, with physician approval
