Whether the patient is suffering from a minor illness, recovering from a surgical procedure or a chronic condition, home health care helps people remain at home and live as independently as possible. Our clinicians work with the care provider, patient, and patient’s loved ones to determine reasonable goals and develop a plan to meet those goals. Our team also provides education about the disease process, symptom awareness and management, medication management, and healthy lifestyle choices to decrease emergency department visits and hospitalizations.
